Healthcare professionals we work with
Every part of healthcare has its own way of working and being paid. As a specialist medical accountant, we work with all of these.
Locum doctors
Locum GPs and hospital locums working sessions across practices and trusts. We handle your Self Assessment and NHS Pension forms for locum work.
Consultants in private practice
NHS salary plus private practice fees. We report both correctly, plan for payments on account and advise on whether a company suits your private work.
Dentists and dental associates
Most associates are self-employed. We deal with your share of fees, lab costs, NHS Pension contributions and practice expenses.
Nurses and aesthetics
Agency and bank nurses with private work, and nurses running aesthetic clinics. We check the VAT position on cosmetic treatments.
Aesthetics clinicsPharmacists and allied health
Locum pharmacists, physiotherapists, radiographers, optometrists and other HCPC-registered professionals working for several employers.
Clinic and practice owners
Running a clinic with staff and premises? We handle payroll, VAT and company accounts for healthcare practices.

Tax rules for healthcare professionals in 2026/27
The rules that most often affect doctors, dentists and other clinicians.
Pension annual allowance
Growth in your NHS Pension counts towards this limit. It tapers down to £10,000 for adjusted income above £260,000, and exceeding it can create a tax charge.
NHS Pension for locum GPs
Locum GPs can pay into the NHS Pension on their locum income by completing Locum A and B forms for each practice and month.
Losing your Personal Allowance
Your Personal Allowance falls by £1 for every £2 of income over this amount, creating an effective tax rate of around 60% on that slice.
Medical care
Care provided by registered health professionals is usually exempt from VAT. Cosmetic treatments that aren't medically needed are usually standard-rated.
Payments on account
Private or locum income can create payments on account towards next year's bill, which often surprises newly self-employed clinicians.
Making Tax Digital
Self-employed clinicians with qualifying income over this amount must keep digital records and send quarterly updates.

What healthcare professionals can claim
Many clinicians miss costs they're entitled to. These are the ones we check for every client.
| Usually allowable | Usually not allowable |
|---|---|
| Registration fees, such as GMC, GDC, NMC and HCPC | Fees for bodies not on HMRC's approved list |
| Royal college and professional body subscriptions on HMRC's approved list | Social membership fees |
| Medical indemnity and defence union fees | Personal health or life insurance |
| Travel between practices, hospitals and patients' homes | Regular travel between home and your usual workplace |
| Courses and CPD that keep your existing skills up to date | Training for a new qualification or specialty |
| Medical equipment, stethoscopes and protective clothing | Everyday clothing |
If you're employed, some of these can be claimed as tax relief on your salary. We'll check both your employed and self-employed income.

Healthcare Professional FAQs
Do doctors need to file a tax return?
If all your income comes from an NHS salary taxed through PAYE, you may not need to. You usually will if you have locum or private practice income, earn over £150,000, need to claim a pension tax charge, or have other untaxed income. We'll confirm what applies to you.
Are locum doctors self-employed?
Most locum GPs are self-employed and file a Self Assessment return. Some locums are paid through agencies or trusts under PAYE, and others work through their own company. The way you're engaged decides how you're taxed.
Can locum GPs pay into the NHS Pension?
Yes. Locum GPs can contribute to the NHS Pension Scheme on their locum earnings by completing Locum A and Locum B forms. Keeping these up to date protects your pension and makes your tax records easier.
What is the pension annual allowance charge?
If the growth in your pension savings in a year exceeds your annual allowance, currently £60,000 for most people and lower for very high earners, a tax charge can apply. NHS clinicians can sometimes ask the scheme to pay the charge from their pension. We'll check your position.
Do healthcare professionals have to charge VAT?
Medical care provided by professionals on a statutory register, such as the GMC, NMC or HCPC, is usually exempt from VAT. Cosmetic treatments that aren't medically needed are usually standard-rated, so aesthetic practitioners may need to register once they pass the threshold.
Can I claim my GMC or NMC fees?
Yes. Registration fees with bodies such as the GMC, GDC, NMC and HCPC, and subscriptions to approved professional bodies, are usually allowable. If you're employed, you can claim tax relief on them against your salary.
Should I use a limited company for private practice?
It can reduce tax for some consultants and clinicians with significant private income, but income paid to a company doesn't count towards the NHS Pension. We'll compare the options using your figures before you decide.
How much does an accountant for healthcare professionals cost?
It depends on how many income sources you have, whether you run a practice with staff, and the services you need. We charge one fixed monthly fee, agreed before we start. You can choose a ready-made package or build your own.
